New Patient Information

At Meridian Behavioral Health, we work with you to achieve your mental health goals. If you are interested in becoming a new patient at Meridian, first of all, welcome! Here are a few guidelines to making your first appointment.

To begin, please check our Insurance page to make sure you will be covered at our practice. If we don’t currently take your insurance, there is a self pay option. Please also make sure you have your insurance card on hand, as we need the identification number and group number (if applicable) to enter you as a new patient.

Secondly, we ask that all new patients keep a card on file. This can be credit, debit, or an HSA/FSA card. Missed appointments will be charged $60 for therapy and $75 for medication management. Make sure you have your card information handy as well.

Thirdly, read about our various providers on our Louisville or Lexington pages. Each provider has a their bio posted to help find the perfect fit for you. If you are interested in the first available and aren’t too picky about the provider, that’s fine too!

Finally, give our office at call at (502) 409-6993 for Louisville or (859) 309-2655 for Lexington. We will take your information over the phone and enter you as a new patient! Our administrative staff will do their best to get you scheduled at the most convenient time for you.

Please Note: We currently accept most Medicare supplements, please inquire with our staff for more details.

SELF PAY OPTIONS: If you are not currently covered by any of these plans, we do offer a self pay option. We can take cash, credit, debit, or check. Please contact us for self pay rates.
